Recovery Assist
Recovery Assist is a program designed to make the transition from hospital to home more comfortable for patients who continue to receive clinical treatment in their own home.
If it is appropriate for you to leave hospital early Recovery Assist will work with staff from selected participating hospitals, and arrange services such as nursing care, wound management, equipment hire or home help.
As a program participant you are assured of the support and care needed to help you on the road to recovery.
Who can use this service?
To be a Recovery Assist participant you need to:
- have hospital cover with Defence Health and be eligible for benefits in relation to the service for which you have been admitted
- be admitted to one of our selected participating hospitals such as a private patient
- elect to go home early, and
- have clinical treatment continue within your own home.

How does Recovery Assist work?
Firstly, before you leave hospital you will need to consult your doctor and hospital staff to determine whether this program is suitable for you and which support services you may require.
With your consent the hospital will contact a Program Coordinator with details of the services that need to be arranged for you.

What happens next?
Once you are home, registered service providers will visit you and liaise with your doctor about how they can best assist your recovery.
The Program Coordinator will also contact you regularly to ensure that all support services are in place and meeting your needs. These coordinators are experienced registered nurses with knowledge of community services and how they work.
