The easiest way to join Defence Health is by applying online. Before you start, just go through this list and check that you have all the information.

Permanent or Reserve (active or standby) member of the ADF
- PM Keys
- Enlistment date
- Discharge date (if former member)
Public servant employed in the Department of Defence or Defence related organisation
- PM Keys
- Employment date
- Termination date (if former servant)
- Department of Defence related organisation
Defence contractor employee
- Employment date
- Termination date (if former contractor)
- Contractor company name
- HR details (contact name and number)
- Details of personal involvement in supplying goods and services to Defence

Switching funds?
If transferring from another health fund, then you need to download, complete, sign and post the Cancellation and transfer authority form to Defence Health as soon as possible.
Cover commencement date
Your cover can start today or you can select a date up to 2 months from today.
Direct debit and/or credit card details
- For the first month's premium, we need your credit card details (card number and expiry date)
Note: First payments will be drawn on the day the application is processed.
- For ongoing payments via direct debit , we need the BSB, account number and account name
Claiming the Federal Government 30% Rebate as a reduced premium?
To claim the 30% Rebate, you need to download, complete, sign and post the 30% Rebate form to Defence Health as soon as possible.
Cooling off period
If for any reason you change your mind within the first 30 days and have not made a claim, simply write to us and we can cancel your membership and refund any premiums you've paid.
