Transferring from another fund
If you want Defence Health to arrange the termination of your membership with your current health fund then you need to download, complete, sign and post the Cancellation and transfer authority form to Defence Health. Your current health fund will then supply us a Transfer Certificate to show proof of your previous cover. All you need to do is cancel any automatic payments you may have with your previous fund.
When you transfer to Defence Health from another health fund we will honour any waiting periods that you have served with your previous fund for camparable cover, provided we receive a Transfer Certificate and you have transferred within two months of ceasing your membership with your previous fund. If you choose to upgrade or change your cover, you may need to serve waiting periods for the upgrade.
Claiming the Federal Government 30% Rebate as a reduced premium
To claim the 30% Rebate, you need to download, complete, sign and post the 30% Rebate form to Defence Health.
Cooling off period
If for any reason you change your mind within the first 30 days and have not made a claim, simply write to us and we can cancel your membership and refund any premiums you've paid.
