Terms and Conditions: Corporate Partners Acquisition Gift Card Offer
The below Offer is provided in conjunction with those employers having entered into, and remaining bound by, a signed agreement with Defence Health (Corporate Partnership Agreement). Defence Health makes the Offer available to Eligible Employees (as defined in the Corporate Partnership Agreement) that meet the further criteria below.
This documents the terms of the offer that will be provided on the below conditions (the "Offer"), which will be available only to Eligible Customers (defined below) that join Defence Health with an Eligible Policy via phone when they are an Eligible Employee during the term of the relevant Corporate Partner Agreement between their employer and Defence Health (the "Offer Term").
Eligible Customers
"Eligible Customers" are only Eligible Employees who:
1. during the Offer Term purchase an "Eligible Policy" via Defence Health's call centre (i.e. via telephone);
2. have an Eligible Policy that has been active for a continuous period of 60 days from the date it commenced ("Qualifying Period"); and
3. at the time they purchase an Eligible Policy, they:
a. have not been insured under a private health insurance policy issued by Defence Health in the 12 months prior;
b. are an Australian resident;
c. do not reside in WA or Northern Territory;
d. are at least 18 years of age.
4. have not utilised any other discount or promotion issued or provided by Defence Health within the 12-month period before purchasing the Eligible Policy (excluding any Age Based Discount or veteran discounts); and
5. do not have an arrangement with their employer for that employer to contribute to, or otherwise subsidise the cost of their health insurance premium under the terms of the Corporate Partner Agreement.
Eligible Policy
The Offer is available to Eligible Customers that take out one of the following products during the Term:
1. Defence Hospital Gold
2. Defence Hospital Advantage Silver Plus
3. Defence Hospital Silver Plus
4. Defence Hospital Bronze Plus
5. Defence Hospital Basic Plus
An Eligible Policy does not include the following products:
1. Any policy with products not in the list above;
2. Any policy which holds only an Extras product;
3. All Kids Only policies (including those which cover Dependent Children or Dependent Student or Dependent Non-student only)
Gift Card Terms
1. All Eligible Customers will be entitled to receive a digital Gift Card valued at:
a. $200 for an Eligible Policy with one (1) adult (Single Policy or Single Parent Family Policy or Single Parent Family+ Policy); or
b. $400 for an Eligible Policy with two (2) adults (Couple Policy or Family Policy or Family+ Policy), from third-party provider Prezzee Pty Ltd (ACN 602 963 422) (each a “Gift Card”).
2. The Gift Card is subject to the provider’s terms and conditions available at https://prezzee.com.au/en-au/policies/terms-of-sale, which you acknowledge will at all times apply in parallel to this Offer.
3. Defence Health is not responsible for any issues with, faults in, or lost or stolen cards once issued.
4. Eligible Customers that take out an Eligible Policy during the Offer Term acknowledge that Defence Health may be required to provide personal information to the provider to generate the Gift Card.
5. The Gift Card, including instructions on redemption, will be delivered electronically to the email address recorded on your membership. Members should check their junk, spam and promotional email folders before contacting Defence Health regarding non-delivery.
6. The provision of the correct email address is required for Defence Health to fulfil this Offer. Your email address can be updated via phone or through the Member Portal. If a valid email address is not provided to Defence Health, the terms of this Offer no longer apply and Defence Health is not obliged to provide you with a Gift Card.
7. The Offer is not redeemable for cash and cannot be returned for a refund.
8. Defence Health reserves the right to substitute the Gift Card with an alternative reward or gift card of equal or greater value where reasonably required.
General
1. This Offer is available only to an Eligible Policy purchased by an Eligible Employee directly from Defence Health via telephone.
2. This Offer is limited to one (1) Eligible Customer per Eligible Policy.
3. This Offer is not available through price comparison sites or certain sales channels other than via telephone.
4. The Gift Card will be sent to Eligible Customers within 30 days after the end of the Qualifying Period.
5. Defence Health reserves the right to amend, suspend or withdraw this Offer at any time where reasonably necessary.
6. Defence Health reserves the right to verify eligibility and refuse, cancel or recover the Gift Card or any alternative reward where fraudulent, misleading or ineligible activity is identified.
