With hospital cover, you can call us for a Hospital Eligibility check, where we can:
- Check that your procedure is included in your cover
- Look for any waiting periods on your cover
- Make sure your policy is financial
- Identify potential out-of-pocket costs upfront
- Maximise your benefits by choosing Access Gap providers and Agreement hospitals
Additionally, if you have any pre-existing conditions, complete the Pre-existing condition claim form to check whether you’re covered for an upcoming treatment.

How to claim
To submit a claim after hospital admission, your claim must first be processed through Medicare. Once this is done, please send us your ‘Medicare Statement of Benefit by EFT’ in the Defence Health claiming app (Snap & Send). You can download this document from the inbox in your myGov account or request a copy from Medicare. We’re unable to process your claim without this.
If you are not admitted to hospital, you cannot claim with private health insurance. This is referred to as an outpatient service, and can only be claimed through Medicare, if you have a Medicare item number.
